Job Purpose

• Coordination of all work streams and change programmes across Adult Services
• Ensuring services are commissioned effectively leading to safe, cost effective services providing choice and control (domiciliary care, care homes, day service and extra care)
• Ensuring that the workforce works consistently, with a common approach (regardless of which team they work in) to enable and empower people and families to be as independent as possible and that people maintain and develop skills and embed a progressive approach to support planning
