Job description

This role will involve:

  • To participate in a full and flexible manner in the provision of services to vulnerable adults with complex mental health support needs and their carers, using appropriate resources of the Council, the Health Trust, the local community, non statutory organisations and primary care.
  • To participate in the Approved Mental Health Professional centralised and out of hours rota, ensuring that an effective and sensitive service is provided at all times, with full documentation logged and distributed as appropriate.
  • To maintain a high standard of record keeping and ensuring that deadlines are met within the time frames for reports.
  • To carry out joint work with local trust including home visits, office visits including meetings and other as required and offering appropriate advice on possible outcomes in line with the Mental Health Act. On occasions this may include travelling outside of the borough.
  • To offer practical advice to vulnerable adults and their carers on a range of matters including housing, health and welfare rights etc - ensuring that referrals for further Care Act assessments or specialist advice is sought.
  • To liaise and consult with service users, carers, and colleagues based within LBN, local trust and with a range of statutory and voluntary agencies to ensure provision of appropriate services to service users and carers.
  • To maintain close links with hospital, care facilities and other stakeholders to ensure that good communication is established at all times.
  • Practice as an AMHP and maintain the necessarily training hours - 18 hours per year.
  • To attend and participate positively in team meetings and AMHP forums with a view to promoting the development of an effective service.
  • To work in close consultation with a range of workers internally and externally of the service, in determining an accurate and sensitive assessment of service user needs.
  • To identify needs and potential vulnerabilities of children. To refer to Children's Services as appropriate and to liaise with the Children in Need Team/AMHP Senior practitioner or Team Manager.
  • To follow local Safeguarding Adult Procedures and ensure that actions are recorded and adhered to.
  • The ability to provide information, advice, support and guidance to service users and carers to enable vulnerable adults to improve outcomes through informed choice.
  • Demonstrate the ability to apply and effectively use theoretical frameworks, social care policies and research evidence.
  • To be aware of and to comply with Health and Safety at work guidelines.
  • To participate in an annual appraisal and takes steps to complete the action plan.
  • To record statistics relating to MHAA's and preparation of activity reports.
  • To ensure that service users have access to translation and interpretation services, and that Braille and signing facilities are made available as appropriate.
  • To maintain at all times a courteous, helpful and polite response to all stakeholders.
  • Demonstrate an ability to use well developed communication skills to present complex/sensitive information in an understandable way, to a range of audiences.
  • To ensure that services offered are responsive to the needs of all users, including their sexual orientation, religious, cultural and linguistic needs, and the needs of users with disabilities.
  • To coordinate Mental Health Act Assessments including liaising with Police, Doctors, and other relevant agencies.
  • To attend Court to make S135 applications including preparation of any necessary reports.
  • Liaise with relevant legal representatives following discussion with a Senior Staff Member.
  • Ensure any incidents and near misses are recorded and responded to within the agreed procedures.
  • Recognise the sensitivity of high profile or complex cases to include notifying the Line Manager of any potential cases.
  • Such other duties, within the competence of the post holder, which may reasonably be required from time to time.
