Job Purpose

The post that requires cover is a Mental Health Social Worker in an integrated community mental health team in South London. This post holds a responsibility of care coordinating, under the Care Programme Approach, a complex caseload of around 25 clients, who suffer from severe and enduring mental illness. The post is essential in terms of delivering care management responsibilities.
