Advanced Social Work Practitioner - Worcestershire County Council - Transitions Team - £28 + Negotiable!

Full time Advanced Social Work Practitioner (ASWP) within the Young Adults Team (Transitions). This is a great opportunity and requires someone with skills across Adult Services and Children's Services.

The Young Adults Team work with young people with disabilities aged 16-25. The work is primarily with young people with learning disabilities, but also some young people with a physical disability or complex health needs, to transition from Children's Services into their adult life. The work is very varied and requires working in a multi-agency manner with Worcestershire Children First (WCF), Special Educational Needs Department (SEND), health colleagues, schools and colleges to ensure young people get the whole range of options they deserve when moving into adulthood. The work involves using Liquid logic in both the adult and children's systems, and using both children and adults legislation effectively.

ASWP's will have line management responsibility for a group of Social Workers/Social Care Workers and will be providing regular Supervision. There is an expectation that when required you will Chair Best Interest Meetings, so an understanding of the Mental Capacity Act and the role of the Court of Protection and Public Guardian is required.
