The Hospital Pathway Team works in partnership with Sutton Health and Care Partnership to ensure timely discharge from hospital and to prevent admissions.

The Team provides a social work service to all Sutton residents in all hospitals, providing holistic, strength based assessment to support discharge home, and to review individuals following a period of reablement. The team works closely with our in-house reablement team which is part of the Sutton Health and Care Partnership providing an excellent multi-disciplinary service to enable individuals to reach their full potential. Full eligibility assessments are completed for individuals to identify long term needs once their independence has been maximised.

Our client is looking for an experienced assessment officer with a range of experience within adult social care. You should have experience in undertaking Care Act Eligibility Assessments, Support Plans, Carers assessments and hold the Trusted Assessor qualification to enable you to do basic occupational therapy assessments.

We require someone who adept at working in a fast past environment, responding to crisis and incoming work. It is essential that you have a strong ethos of working in a person centred and strengths based way. There is a wide range of work involved in our team which makes this post interesting and varied.
