You will work in line with the Adult Operations Charter promote the vision of ‘Helping People to help themselves, making personalisation real'.

The position covers the wide geographical area including Canvey Island, Hadleigh, Rayleigh, Rochford, Great Wakering, Hullbridge and Hockley. It is a medium sized team with one team manager, 2 senior practitioners, 8 social workers and 9 social work assistants.

The role: 
• To support service users from the age of 18 and over who have a variety of needs and across all specialisms. 
• To complete assessments, statutory reviews, and MCAs
• To undertake safeguarding, carers' assessments and continuing health care assessments.
• The hospital team receives a very high number of Section 2 referrals and is subject to the Delayed Discharge Reimbursement Act 2005.
• The team also has a high referral rate of safeguards and often these will involve working alongside the police and health partners. 
• Team members are supported and given the opportunity to complete safeguard investigations, continuing health care assessments and undertake other roles as valued members of a multi-disciplinary team. 
• This University Trust, in addition to the usual medical intervention wards, specialises in Cancer and Stroke Care and staff are encouraged to enhance their learning in these areas to support safe and timely discharges. These often encompass cross boundary and partnership working due to the intake of patients from other areas.

Essential requirements:
• Diploma / Degree in Social Work, CQSW, CSS or equivalent
• Current HCPC Registration
• Evidence of continuing professional development e.g. MCA BIA AMHP
• A good working knowledge and ability to use information technology and related systems
• Post qualified relevant experience in Social Care setting
• Evidence of working with a complex caseload that is underpinned by evidence based practice
• Required to be mobile throughout an operational area. To be able to attend meetings or provide cover in other locations. Driving licence and car or access to other effective transport arrangements is essential.
